Transformer and Partial Discharge Monitoring

For 138kv/13.8kv 24MVA transformer I want to know if there is a partial discharge monitoring to be installed on the transformer and monitor the health. I am wondering such device is installed when the transformer manufactured or could be re-trofited at a later time. what about for 15KV switchgear. Can someone brief me on details or if there is a good web site. I appreciate
