Discovery's Foam Loss: Human Error

A NASA investigation team has concluded that the chunk of foam that detached from space shuttle Discovery during its July 26, 2005 flight, occurred due to inadequate methods of applying and repairing foam on the space shuttle's fuel tank. Well, they don't put it in such certain terms. Read for yourself.
