I was invited to join our Safety Committee to conduct an investigation concerning an employee's back injury while connecting a 10" hose from a gasoline barge to our dock manifold at an upstate NY petroleum terminal. The plant operator pulled a muscle while pulling up on a ratchet and socket "torque-ing" one of the bolts on the flange. The bolts (still in use) have steel rods welded to the hex heads to keep them from turning so the men need only one wrench. I said welding on grade bolts would void the original manufacturers certification but more importantly to us, they could fail and the pollution to the river and possible fire hazard.... Well, the company's contractor that did the welding says it is true that they could fail if they had been tempered bolts but these are soft iron bolts. What?! I have expressed my concern much to the dismay and disdain of management and now they told me they were looking into A307B bolts and ANSI B16.5... ...am I being unreasonable in thinking just because we haven't had a failure in years of use and hundreds of petroleum barges and products with these bolts...the risks....
