Forsmark 1 Incident - July 25, 2006

In 400 KV switch yard of Forsmark-1 NPP (Nuclear Power Plant) of Sweden, During operation a "ground current" protection breaker wasn't blocked and resulted in an electric arc when the section disconnector was opened, consequently Short circuit between two phases occurred.

How such an event can occur in HV switch yards ?

Although this looks like "homework", I'll take a stab at the most likely answer in two words:

Thanks Tom,

You are right.

It is written in the report of Task Force Report "Defence in Depth of Electrical Systems and Grid Interaction" of OECD NEA CSNI (NUCLEAR ENERGY AGENCYCOMMITTEE ON THE SAFETY OF NUCLEAR INSTALLATIONS) the event is because of mistake of maintenance personel after a maintenance.

My question is not a homework question. I was expecting a technical explanation for electromechanical event which caused the short circuit.
