Steam Turbine Failure Mode and Effect Analysis

haaaiii...my name is nico...i'm new in this forum...

i'm undergraduate students and now i'm doing my final project...

my topic is making failure mode and effect analysis about steam turbine....

i wanna ask about steam turbine failure mode, the effect and the recommended action to prevent this failure....

no...i don't mean some one works for me....

i've made the analysis but i don't know the real case from the turbine...

i only read from the book and other source....

so i want to ask about the real case that occur in industry....

A failure modes and effects analysis (FMEA), is a procedure in product development and operations management for analysis of potential failure modes within a system for classification by the severity and likelihood of the failures. A successful FMEA activity helps a team to identify potential failure modes based on past experience with similar products or processes, enabling the team to design those failures out of the system with the minimum of effort and resource expenditure, thereby reducing development time and costs. It is widely used in manufacturing industries in various phases of the product life cycle and is now increasingly finding use in the service industry. Failure modes are any errors or defects in a process, design, or item, especially those that affect the customer, and can be potential or actual. Effects analysis refers to studying the consequences of those failures

As we studied in the years past and practice (and teach the new entrants to our organisation)

In FMEA- Failure is not the failure of equipment (why turbine fails to start-up). failure is the failure in the system.

eg Low oil level in governor - and the effect may be multiple due to this, for each you have to calculate the criticality index (RPN) and then block the loop holes depending on that.

yes but it's hard for me to know the real case...

i only read from the book and web...

do you have some suggestion for me to find another example?
