I don't believe that you are at all sorry for being vague. I believe that you're afraid of looking stupid when you certainly shouldn't. Nobody knows everything. But you're not helping yourself by only posting the information you don't understand but nearly none of the information you do understand. From what you posted I do not know if your panel looks like this

or this

or this

or thousands of other panels that are on or near something made by Siemens. The way you've written your question I cannot even tell if these writings are inside a panel, above a panel, on the schematic for the panel, in a chassis next to the panel, on a component in the panel, scroll displayed on a human to machine interface of the panel or if this information has anything at all to do with a panel. We have no idea where this panel is or what it might be attached to.

Far too often when people come here with "wot dat?" questions, they never answer any of our questions that might help us understand what is happening. I am tired of this and I am taking it out on you. I know that this is not fair but you've provided one of the least informative yet highly specific questions I've seen in a long time.

I'm not going to use one of my 20 questions in trying to help you. Tell us what you do know about this panel, not what you don't know.

Never heard about mobility index and service index but for form factor Refer IEC 60439.

Please refer to IEC 61439. Form 4b has got seven sub-classifications.
