Lots of that going around. Most famously, Facebook. One moment, you're happily using it flawlessly and then all of a sudden, you're looking for something where it used to be.

Miscommunication: when what people heard you say differs from what you said. Make yourself understood.
I started to reply to Vulcan that there was no "person icon" in the upper-right of my window. Then I noticed a tiny mark that looked like a defect to the right of the search icon. I next realized that it was a bit of the "person icon", and that there is a scroll bar at the bottom of the window. At the normal window width that I'm accustomed to using, that "person icon" is not visible. Widening the window made it visible. Of course I could also have used the scroll bar, but then I couldn't see the left side of the page.
It took me a while to realize that a circle and an arc represented a person!
I consider this to be poor programming of the website. At least the header of the page should be visible at any reasonable window width.
