Is it possible to connect an A/C directly to a PLC via transformer?

I assume you mean air conditioner? If so, you could not run one directly from a PLC, but you could certainly control it. Why do you want to air condition heart attack patients? It would take a long time to produce any noticeable cooling in the blood. You could always pack crushed ice in their arm pits and groin - that would cool them quickly. And would make life-long friends.

Can or cannot depend on your plc interface and function.
