what is the reason for frequent voltage fluctuation in our homes?

whether there is any remedial measures to avoid voltage fluctuation?

There are lot of reasons for voltage fluctuation, for example, the bad grid environment, electricity generating station, other power users and so on. Try to determine when it happens. A/C starts or some other load comes on. If so that is not uncommon. However you may have a loose connection in your service conductors or in your panel. Call you utility and have them check their connections.
