why we use the single phase isolation transformer in the operatio theater

Because the human tissue without skin has a resistance of perhaps 100 Ohms. Thus, even a few Volts can prove lethal. But, the local hospitals here tend to use very fancy GFI's.

You also have to be very guarded about any possible spark in the presence of high oxygen concentrations. A flash fire in the patient's lungs is always difficult to explain.
