For large branches, the pad plate size also will be large. To ensure that the pad plate works with the main line properly, it will be necessary to do plug welding on the pad plate at different places. What is the limit of pad plate width, beyond which the pad plate should be plug welded in addition to the welding on inner and outer diameters? Any guide lines?

WOW!!!

This is a new one on me. I have never seen a reinforcing pad on a piping stub-in branch wit "plug welds".

Where did you see that plug welds were required?

As for the width of the Pad. The pad with is normally one half the diameter of the branch pipe.

Example: If you have a 10" (DN250) branch on a 20" (DN500) header then the pad with would be 5" (127mm) wide.

Plates have tolerances on thicknesses. Pipes have tolerances on OD. Hence it is logical to expect that the pad plate may not hug the pipe properly to offer as a single body for hoop stress or other stresses due to the weakening of the pipe due to the hole of the branch. Hence I visualise the need for plug welding. Probably this aspect is not covered in any code.
