The boxes didn't seem to be too heavy the way the warehouse employees were pulling it off to get to the driver.

That's a perfect example of why an OSHA inspecter will look at the scratched paint on a forklift to determine if the company is observing a diligent safety program.

I would be interested to know if the driver went through forklift safety training.

The driver of that incident will be held accountable unless there is no record that the company provided him training.

He might be related to the guy in the other video that comes up in the link below. I have lifted loads that weigh less than the max load but the load center is way out there. One very expensive welding robot was almost an insurance claim. It arrived on a tractor trailer. A test lift of 4" above the trailer floor looked solid and safe. I had a helper try rocking the load to see if it was stable. I backed the load away from the trailer very slowly without any problems. When I lowered it to the ground all seemed well. At about 20" above the ground the forklift tipped forward. I was able to lower the mast and set the robot flat on the ground with out damage. I was unable to transport the robot to the building without dragging the shipping pallet. All I can figure is that when the mast was lowered the load center moved out and away from the fork lift as the mast was tipped back as far is it could go. Yes I had the seat belt on.

Wow he has very slow reactions, If he had dropped the mast he would have got away with that.

Did you notice he walked part way round the back ?

He should have used the seat belt. Instead of throwing his arms forward to break his fall he could have reacted to the situation better. Or at least he would not have slipped out in the possible pinch point area outside the operator area.

Peter , had to add this; I remember picking up a re-stacked pallet not being aware of the additional weight. When the pallet was about 6 inches off the floor the forklift came up in back and I was fast to drop the mast . When the back end came down it came down fast my body didn't and the overhead guard connected with my head. I still have the lump there, fast reactions and all. If I didn't panic and let it down slowly it would have been a controlled lowering and the gap between my head and the overhead guard might never have closed so quickly.

That was a small forklift to begin with.

Every inch the load is away from the mast, you can deduct 100lbs from the maximum permissible load. That is why a forklift with side shift has a slightly lower max permissible then than a same model forklift without it.

That is why forklift companies will not certify extensions for a forklift.

That forklift was probably rated at around 3000lbs and that load was not only about 10 to 12 feet away from the mast but he had the mast tilted all the way forward and raised. That operator is lucky he wasn't hurt.

Look close and the bottom row has been hit by forks before he started the lift. I am curious if he was the one that hit the stack to begin with or if he was stuck working with a mess someone else created. It did not look like he had much protection from falling glass.

I don't think that was forklift operator error. He was following instructions from a spotter who laughed at the incident.

I think that is more of a product stacking error. I think maybe the company should cut those stacks to at least 2/3 of what they are currently doing. Especially with their product being glass.
