Most modern drawing packages have symbol libraries that can be populated either by the software supplier or by the user. It's then a case of selecting standardised symbols for plant items, editing them, and then "joining the dots".
Anyway, the crafting of the P&ID is the job of the Process Engineer and the Controls & Instrumentation Engineer between them. These individuals dictate what is to be drawn.
The drafting itself is simple. 2D. Monochrome. Standardised layers. All everyday stuff for the competent Drafter, who will soon yearn for something a bit more, well, interesting.
