You need a lot more information. Motor HP, voltage, load etc.

The most probable case is the motor is unable to produce the required torque to run the load. You need a motor of larger capacity for the load.

Or you may have the motor connected incorrectly, i.e. you have connected it in the pattern for a higher voltage than you have. Or you have a bad motor, or you have a blown fuse somewhere, or you have a bad controllers etc. etc. etc.

As was said, you need to provide more details if you want meaningful help.

And you should also tell us if this is a real situation, or a project for your engineering class.

Is there more to the problem you haven't shared with us?

(1) read carefully the name plate of the motor. note down the current, voltage, frequency and kW?

(2) observe the terminals - are there 6 or 3?

(3) check from your purchase document or manual if it is a 3PH squirrel cage induction motor?

(4) check all the mechanical connections to the shaft and ensure that there is no clutch or brake that is stuck or malfunctioning?

(5) measure the no load current in each line using a tong tester when the motor is running without load (presume from your question that it can be run that way)?

(6) read this . . . http://www05.abb.com/global/scot/scot259.nsf/veritydisplay/bf88560fb1b0335cc2256fc6003e4e05/$file/motor%20guide%20gb%2002_2005.pdf

(7) and this too . . . http://www.advancedfluidsystems.com/files/resources/Electric_Motor_Troubleshooting_(Polyphase).pdf

like suggested by my learned friends . . . tell us more about your motor and application . . . many here will guide you well . . .

Is the selection of motor,CB and OL ok?. Is it driven by belt drive, did you check the voltage/current/speed etc at no load and on load?.
