Leave the format up to the testing company. Compressive strength is usually what is tested.

You should look for another job. This one's way over your head.

If this sounds cruel, I'm sorry.

In my opinion following things should be included Date of casting bar bending schedule checked with site engineer signature method of curing signature of erection side (mechanical engineer )for verification of pocket detail or bolt detail along with copy of drawing. If possible type and grate of cement used with batch no. proportion used or name of concrete supplier and grade of concrete supplied.with suitable documents from supplier side. this is very elaborated list and according to importance of work some details can be omitted. I hope this can help you. thanks pravin B.E.civil

These are the things you need to do before and during the placement of a concrete equipment foundation:

1. A testing agency needs to conduct soil compaction tests. Compare the text results to the specification requirements to determine adequacy, and if they are not, ordering further compaction and other corrective measures.

2. A surveyor or the Contractor to verify the elevations of compacted gravel or soil surface and the top of the forms throughout the foundation area. Compare the results of the survey shots to the elevations shown on the structural foundation plans.

3. A visual inspection of the steel reinforcement and formwork installations to assure compliance with the Contract Documents.

4. All Field Observation Notes/logs by the Resident Engineer and others, and Construction Inspector's Field Notes/logs.

4. Items that the testing agency has to do while the concrete is being placed, in accordance with the project technical specifications:

a. Slump Tests.

b. Air Entrainment Tests.

c. Concrete Temperature readings.

d. Prepare several concrete cylinders to be used for compressive strength tests later at designated time intervals, in accordance with the project specs. The cylinders must be protected during the curing process.

e. Gathering of each concrete "batch ticket" for each and every ready-mix concrete truck. The batch tickets must include the weight of each aggregate used and it's moisture content (from field or lab tests), the weight and type of cement used, the amount of the water used (weight, not volume), list of all admixtures and liquid volumes, dry ice weights and volumes used (if needed), temperatures of each material used in the mix, ambient air temperature, time when the batch was made, the ready-mix truck number and company name, and the project name, project number and location.

All of the above must be included in your report(s). This is just a start for you to work on. Typically Civil Engineers do this work, not Mechanical or Chemical Engineers. If these procedures are not delineated in the Project Manual, then you and the Engineer of Record are in big trouble....the project is most likely doomed! That's my take on the matter.
