In fixed output IC engine, it is fixed by design. Various methods are available to ascertain AFR for a given IC engine. In variable speed engine, you control the AFR by accelerator.

Generally speaking, a/f ratio is controlled with either a carburetor or fuel injection. Discussion of the details (of either method) can fill volumes.
