The cable drum of material bucket was broken. Please advise the probable cause/possibilities of the damage.

When you are working with poorly maintained equipment or over stressed equipment it is very important while it is being operated to stand very close to the equipment so that when it fails you will be able to record even for a split second what actually happened.. Hovering over it seems to be the best position.
