Yes, it happens. No, it is not desirable. Lastly, it does not require a neutral for the imbalance to occur.

Yes, there is a huge voltage difference between phases and neutral.

How about current?

Yes it will happen. So check your cable size, total connected load on each phase, Cable length and distance from the main power source and other factors like type of cable used, any cables joint and the voltage drop.

It will happen if your cable is running a long distance.
