Honeywell Experion DCS Analog Output Card Failure Issue

In Honeywell Experion DCS with 4 C300 Controllers and Series C IOM's,Recently a issue was faced that one IO Module which was Redundant Analog Output Module failed and all the output went to initialization states...We checked from the DCS Server, and found the points analog output blocks gone inactive. We then activated the blocks one by one to restore it back. Later we also found that soft AO configuration was also changed as some of the valve were opened in field and DCS was showing 0% so we had to change action again to "reverse".

Please help me out what can be the cause of the AO card failure?Why the point AO went to inactive state?Why the action changed for some valves in configuration?
Please help me in this regard.

The module configuration is held in some form of non-volatile memory. The data in this has been changed. The most likely cause is electrical noise, either conducted or radiated.

Review your earthing/grounding scheme, and check that all possible sources of radiated electrical noise in your system are adequately shielded or otherwise protected.

- Residual latent ESD electrostatic discharge damage to a semiconductor.

- Failure due to overheating, like panel cooling failed.

- Malicious reconfiguration by an insider with access to configuration.

- Hacked by an outsider.

Have you thoroughly reviewed the Network History Module to identify which circuit caused the failure?

If the DCS system is properly configured there should be a "major-fault" recorded inclusive of the A/O address along with DATA identifying the type of fault.

My first guess is that you suffered a short-circuit and/or ground and/or improper voltage was applied to a field device with the cause attributed to a person troubleshooting, tuning, or adding a device.

My second guess would be that a person inadvertenetly removed an A/O card and/or inserted an A/O card incorrectly.

My last guess would be that somehow a lot of noise was induced into the DCS system causing the program checksum to be invalid.

These types of fault will cause the DCS to isolate the A/O section in order to protect the rest of the DCS system.

If you do not have the technical expertise required to perfrom this, call the factory

@all:Thanks for your valuable feedback's on this.
Apart from this, on investigation the DCS event also shows the following:"IOM (input output module) switch over to Power ON"
Note: during this incident of Redundant Analog Output Card failure there was a job going on one Control Valve which was terminated on the same Analog Output card. Please help me out what can be the cause of IOM Power ON?

I srongly suggest that you contact the Honeywell Field Services Division and get help with your DCS issue(s).

It is possible to research error code identities by accessing the DCS via the engineering node then review the available error code list.

Understanding, keeping track of, and logging any/all DCS system errors is critical to keeping the system operational and safe.

If you do not understand the error codes and their affect on the system operation, it will not be possible to troubleshoot or maintain the system properly.
