For a high level chemical lab in college, we had someone come in wearing khakhi shorts and sandals. Needless to say, they didn't qualify for having "proper amounts of protection" that day

I had a fellow who had been hired as a steel fabricator (welding, grinding... ) report for his first day in nylon basketball shorts and sandals.

When I sent him home for more appropriate work dress he objected... strenuously. In his opinion the clothing he was wearing was completely appropriate for the tasks.
