Appropriate preventive maintenance techniques are critical to avoiding every imaginable adverse situation, no matter what the industry or application. This will not remove all possibility of those situations, but the odds will be reduced in your favor. This example was of a life-threatening situation; about as adverse as it gets. But even just down-time and lost production mean less profit, and who wants THAT?!? It points out the relationship between QA/QC and safety - if it isn't done safely, it isn't done right, and if it isn't done right, it just isn't safe.

Having had a boss, whose motto was "let tomorrow take care of tomorrows worries" I have full sympathies to the story. By the way, I was responsible, among other things, of the PM of some mainframe computers online 24/7. We had some very interesting discussions thruout the years. While friendly, never saw eye-to-eye on the subject.
