A string or cord (cord may include wire) must pass through that hole. Now is it to wind the thing up, or does the thing wind up the cord?

Moving the cover (lower left post #9) drives the gear train (Shown in post #2) to advance two foil strips (left center #9) while removing the cover strips (middle row, left side #9) which then allows the user to inhale the different powders through the mouthpiece (top row, left #9) into the lungs. The dial in the bottom picture of the original post indicates the number of doses remaining in the contraption.

It's not automatic, but it seems to be overly complex and is probably one reason for the high cost of the drugs inside.

While it is not automatic, it does dispense medication, Tony.

It looks like there is a little swing cover for a hole that you could load or unload something!?!

It also looks like the kind of apparatus found in a gas meter.
