The diaphragm diameter is determined by the force you need to operate the regulator correctly...
For very low pressures i.e. natural gas at 22 milibar, the diameter depends on the flow rate required through the regulator, but can be up to 30" in size.
As for the gas connections, they are determined by the required flow rate as well, they could be M5, 1/8" BSP right up to 2" BSP or on to flanged pipe fittings more

The diaphragm diameter is dependent on several factors.
Primarily it is the volume flow outlet and pressure at the outlet this requirement gives factor which determines the high pressure inlet diameter and its ability to deliver required volume at a fixed delivery pressure or a variable falling pressure if supplied from storage cylinders, this also dependent on gas type ie LPG inlet pressure does not vary much[except for temperature variations] until the cylinder is nearly empty this is relevant to any gas stored in liquid form.
The diaphragm has to be large enough to close the inlet valve at the determined outlet pressure taking into account these factors
