Possibly improper alignment between the pump/motor?

hard starts/stops?

is the coupling properly sized for the application?

improper spacing between the pump/motor?

Most of the failures i have seen are a direct cause of improper installations, but on occasion you might find a faulty coupling/insert from the manufacturer.

Please tell us some details. What material was the spider made of? Was the spider correctly sized for the torques in your application. What is the misalignment? Did you really have cast iron?
