Distribution Transformer - Tap Changer is Burned

dear all

found distribution transformer tap changer was burn at nominal pin for phase A & C, not other electrical equipment damge reported by local utility. i would like to know the reason of burn of tap changer nominal pin and the rest of part in good condition. my ideal is sabotage, but no evidence to prove. any ideal
