There is a spindle and that is driven by a motor through a gear (or other mechanisms).
In the end of the spindle is a taper (and that has to be defined as ISO 50, ISO60, JIS, MT or other standards) In this taper the milling cutter locks. And then as the spindle rotates the so does the cutter. And the the cutter cuts the material.

You put grain in the top and it falls between two big rotating stones through a hole in the upper one. It is ground into flour and exits at the periphery of the stones.
