I don't think I could go off half-cocked and pluck a copy from our quality assurance division here as I would run a-fowl of management (they can be a real mother hen about these sorts of things).

I would go so far as to go out on a wing and say one of the eggsperts here may have a copy of a template (that's no yolk), but in the mean time have you tried your local government energy website for a energy audit template relevant to your country and industry (a business energy audit perhaps)? I would have thought they would have had them, or at least general guidelines or a link to a relevant website, additionally an internet search of "electrical energy audit report" produced numerous examples and guidelines.
