Where:

Ds = o.d. of shell

Dh = o.d. of head

Df = o.d. of flanges

lw = leg of fillet weld of shell or head to flange

Dw = diameter of any flange bolt washers

Ocl = bolt offset from centerline;

Then for sure Ocl ≥ (max[Ds,Dh] + 2lw + Dw)/2; and Df ≥ 2Ocl + Dw.

All of that is pretty easy to figure out by drawing a sketch.

TEMA (Tubular Exchanger Manufacturer Association), ASME (American Society of Mechanical Engineers), or other local standards may apply, depending on where you are. But I too am too lazy to look them up, plus I don't have the books.
