If the injectors are worn, they can "slobber" fuel into the cylinders, rather than producing a well atomized spray. The large droplets can lead to poor combustion, excessive smoking, and cylinders missing. There may be other causes, but that is where I would look first. This would be more likely at low feed rates such as at idle.
