Maybe. I'm not sure what type of test you want done from your un-punctuated description. I can think of a couple of tests that might be what you are talking about. I'll bet you that if you more clearly define this test you need done then you will have produced precisely the format you need.

There's usually a sample form in the appendix to British Standard 7671.
