5 WHY Analysis (RCFA)

Hi there,

I am new to the RCFA (Root cause failure analysis) and that includes the 5 Why analysis method. If you have any useful resources to assist me to better understand this field, can you kindly advice me.

Ask yourself, who, what, when, where and why.

Who should help you, when a simple search, How to Search the Internet SHOULD yield plenty of information?

What will you gain by having others do the search for you, or spending their time educating you?

When will you learn How to Search the Internet and become independent of others for knowledge?

Where have you looked for this information, so far?

Why have you not learned How to Search the Internet?

If there's going to be an analysis...... You to realize that with the information you supplied, we can only push you in the direction......... Because the only way for you to really learn is for you to actually do it and learn the structure for an analysis......

To some it may seem comical or retarded, but in realty, they just have an axe to grind or issues with members here.

But the real reason why we just push you in the direction for this is that You will need to know and understand the foundations of RCFA, otherwise you'll just be parroting, and eventually fail.

GA... I have to agree.

Hurbert.. you did not get this job by accident (did you?)... so I am assuming that you have the qualifications to support your job! That said, you had to study, your job also means you have to study!

No matter how long you spent in class, no matter how many books you read, NOTHING will prepare you for the big, wide world.... I'm guessing you've found that bit out, hence the questions... BUT as in class, folk here will not do the work for you.

Give pointers, make suggestions... and you will make mistakes. I've made plenty, and while I learn from those mistakes, guess what, I still make a few more. That's life.

Now if you come back after finding out your answers, and want a bit of clarification.. ask away.... everyone, including Lyn, will be happy to help.

Sadly, there is a very common mind(less)set among many company managers in this country that promotes inefficiency, waste, and excessive employee stress.

Many people study for years, then work long and hard to gain superior knowledge and skills at their job. They become very good at getting their projects done well and on time. Many get done early and use their time to help/mentor fellow employees with their tasks. This promotes employee communication, cooperation, and job satisfaction (employee morale). Employee efficiency and product quality improve and that enhances the company bottom line (profit).

Many of these (now) highly skilled employees are VERY happy with their current position, pay, and cost of living wage increases. All cylinders are firing and the company engine is purring along nicely.

Then upper level management steps in and thinks it would be a great idea to promote(?) the happy "experts" into other positions, like mid-level management or areas where these employees have NO previous experience or training. After all, they are now so experienced they can do ANY job with no training, right?

Most will struggle for several months trying gain the expected skills that normally take years to acquire and apply effectively. They become highly stressed and unhappy because upper management is dissatisfied with the lack of immediate increases in productivity. To further degrade employee morale, the new employees that were hired to perform the complex tasks of the recently promoted are not performing well. They are also under severe stress to learn in weeks what usually takes years. Everyone is miserable, schedules collapse, and productivity and efficiency take a huge hit. Upper management has NO idea what went wrong. The recently promoted leave, or are let go, and the company has to downsize and "restart" its now crippled engine.

I've seen and experienced this ritual many times over the last few decades and it seems to be getting worse.

The point of this long story is that some of the CR4 questions we see may not deserve ridicule. People who seem completely clueless in their "new" jobs may have been purposely placed in that position through this corporate ritual of self destruction. A little tolerance couldn't hurt when we don't really know the circumstances behind the original post.

With all that said, I do support the more venomous responses when the OP becomes a belligerent jerk or tries to promote over-unity projects and junk science.

I would suggest starting with a simple search on the site. If you prefer printed material, there are numerous books and guides available. One starting book is "The lean Six Sigma Pocket Toolbook" by George, Rowlands, Price, and Maxey (ISBN 0-07-144119-0).
