A shoe leather stretcher so that bunions or differently shaped feet can fit in standard shoes.

You are right.Your comment sent me on a search and I discovered that it is exactly what you said.. a tool used by cobblers to create a space in shoes to relieve pressure from bunions.

To me it looked like an antique tool of some kind but after looking for "bunion stretcher" I see they are still being made and sold today.

I'd say some type of riveting tool or something else requiring a press fit. Because of the presence of the "tightening wheel", some people may not have the grip strength to complete the operation, and/or the parts fitting together have too much variance of force required to do so. Additionally, the wheel may also aid in precision in the mating of the parts.
