Mechanical removal can be achieved by dismantling the engine and carefully scraping the carbon off the metallic parts, then reassembling it. This is sometimes known as 'De-coking'.
Some engine users advocate the 'Italian tune-up'. The procedure is as follows:
Locate a long rising gradient
Locate the vehicle at the foot of the gradient, stationary
Select second gear
Thrash the living daylights out of the engine and drop the clutch
Accelerate at full throttle until the engine is well into the red-line region for a few seconds
Reduce power and resume normal engine operation.

My friend, Bartolomeo Gambino who lives in Cortona, Italy, had been having trouble with loss of power to his 2-stroke Lambretta-powered Carro di Gelato.
In a burst of inspiration, I advised him to try the 'Italian tune-up' remedy.
He had never heard of such named method of improving power, but being a young and keen sort of fellow, was prepared to give it a try.
There were complications in the translation, and poor Bartolomeo started at the top of the gradient, not the bottom.
His unfortunato Carro di Gelato gathered speed very quickly, scattering the ices far and wide, to the delight of the local children, as he accelerated downhill, the brakes of his cart being unequal to the combined effects of gravity plus full powered movement.
Unfortunately Bartolomeo was unable to slow down as a cement mixer truck crossed his path, and only a violent swerve by its driver, saved Bartolomeo from passing beneath, just before it toppled over.
Bartolomeo ended up in the local Hospital, as you can see from this photo he sent me.
Bartolomeo is now asking that intending users of the 'Italian tune-up' method of de-coking an engine to improve its power output, should carefully consider that unexpected results sometimes occur, from not carefully reading the complete instrucziones.
I have forwarded Bartolomeo a box of the finest chocolates available.
He is enjoying the fuss the are making over him, except for this one and is asking if a collection may be taken up, on his behalf, to enable his purchase of a new Carro di Gelato.
