The may depend on specific valve construction and on the characteristics of the fluid involved, especially toxicity.

Most refrigerant valves, for instance, are back-seating, which means that when fully open, the valve packing (or seal) is isolated from the fluid. Thus you can unscrew the packing nut, remove the old packing, install new, and retighten the nut.

In general, you need to know what is isolated from what (inlet, outlet, stem area)when the valve is either open or closed.

Sigh. If you have to ask, you're not qualified to do this repair. Call a plumber. You may have to do this anyway because you may have to replace the whole valve due to a lack of replacement parts or a to badly damaged valve.
