Steel Heat Treat Question

is it absolutely essential to remove the hot rolled surface before heat treating 4140 alloy?

You should ALWAYS remove the hot rolled surface to remove surface cracks, inclusions and laps which can act as crack initiation sites. Doing it before heat treating is easier than doing it after.
