how do you know if the engine fan relay is bad?

Then what makes you think the fan relay might be to blame? It may be a bad temp guage.

Is there sufficient coolant in the system? Is the air temperature in the high 90's? Is this a new problem? How long is too long?

Probably relay may be relaying message to the fan that keep rotating but don't throw air.

the temp gage will shoot up after less than 10 min idling. coolant is full. fan will run when engine shut off.

Actually most of the comments explain why you should suspect an engine fan relay is bad. To know that the relay is bad you must either replace the relay with a new one and see if the observed failure mode (fan operation or over temperature condition) is gone or remove the relay for external (of the car) testing of the relay by seeing if the coil activates the contact and if the contacts support the rated current.
