Oldham coupler, it appears to be. The top image indicates the shafts are not in line with each other.

(I am hoping this isn't homework, I may have chimed in too quickly)

it is not oldham coupling.An Oldham coupling has three discs, one coupled to the input, one coupled to the output, and a middle disc that is joined to the first two.Fig is below

Yes, there are three discs shown in your image. It appears the shafts do not align. This is (appears to be, anyway) an application for an oldham coupler.

Do the input and output shafts align?

No it consist of 2 set of flange and in between them a spider is placed and both the set of flange in different shape.1 set of flange consist of 2 flange and the other set is consist of 1 simple flange and the other one is in convergent shape.

Spider? As in jaw coupling?
