Does it take in account the recent circumference increase in the workers?

That doesn't matter. What matters is that they are strategically placed in a zig-zag fashion so that motorists can not avoid running over them.

And, there must be some unwritten rule that the cover always be at least one inch or more above or below grade, so you will know when you hit one.

I'd like to see one that spins in the air and lands back in place when you run over it.

Manholes or handholes are required to be located so conduits dont exceed maximum NEC allowed 360 degrees bending of conduits in between pulls.

Other than that, their limitations are practical and in your customers requirements:

lbs of pull the cable can withstand without being damaged, lbs of pulling ability of your crew or tugging machine, length of cables on reels, length of fish tape, etc. And usually you dont wanna go up to 360 degrees bend even if it is technically legal. There are no special requirements for MV manhole spacing.

In addition to response # 9 I would suggest you review the local code in your area.

I would also suggest you take into consideration the underground conditions as well as the grade elevations you will be installing the manholes into.

If you are in a high ground water area the manhole locations need to be spaced so as to allow evacuation of the water in smaller quantities so that you limit water encroachment and possible damage as much as possible.

Also; Over a long term period of operation having more manholes on a closer spacing interval will allow faster and easier UG repairs theerby reducing downtime if a fault is experienced.
