Do you mean a natural knee or a artificial (prosthetic) knee? There is a major difference between the two.

Most prosthetic knees replicate the natural knee in function and movement. The artificial ones are not huge door hinges as some people might think. When the prosthetic is implanted (installed) the most frequent way of doing it is to saw the bottom of the thigh bone, femur, off just above the knee. The same is done with the shin bones, tibia and fibula. Since these bones are hollow a piece similar in shape to a large "Thumb Tack" is hammered into the top of the tibia and fibula. This has a relatively flat metal top with a thick Teflon pad secured to it. The same is done with the femur except the bottom surface is more rounded with two distinct hemispheres on it. This is also hammered into the hollow portion of the femur. Prior to hammering an epoxy adhesive is placed on the metal shaft and inside the hollow bone to help secure the prosthetic parts to the bones. Although some of the ligaments are moved aside during the procedure, all ligaments are placed in their original position and the incisions stitched closed. As part of physical therapy there is a small device that the lower leg is placed on and the device extends the angle of the knee. This is increased as the healing progresses.

The model I am most familiar with is the Nex-Gen Knee manufactured by Zimmer Tri-State. After one is implanted the manufacturer usually gives a card to the recipient with an picture of an x-ray of an implanted knee. This is to "prove" that you have one and that is why you are activating the metal detector.

A general anesthesia is normally used. One unusual experience is if a spinal block is used. The patient can feel, by his/her back being pulled on the table, the surgeon tugging on the leg, hear the pneumatic drill, hear the pneumatic saw and everything that is being used, hammering the knee pieces into the bones, etc. as they are done. One instrument, among many, that are sometimes used are vise-grip pliers. Don't recall what they were used for but I saw them there.

This procedure is not difficult to have done. Many people have both knees done at the same time to limit recuperation time and the length of physical therapy.

If the old one is worn our this is the way to get rid of it and have one like the original one. Many people who have one done like the improvement so much they soon have the other knee done. The only difference between the original knee and the prosthetic knee are the materials of construction and who manufactured the knee.
