It might help if you could identify the equipment, the schematic, and the stenciled part number on the PWB.

Most companies use proprietary numbering on their electronic components to prevent aftermarket replacement and/or tampering therefore the chances that identification of the component is pretty much zero.

If you have access to an IC comparator test unit and a new or known-good IC identical to the one in question it is possible to compare the known-good unit to several similar units until a match is found.

It is usually much cheaper to replace the board or send it to the Original Equipment Manufacturer for repair.

With so little info to work with,I can only make a guess, based on the pin out and case structure.

It is probably a precision resistor pack.

Value? Unknown without further information.

You are most likely correct to my mind, though I did not "catch on" till you posted.

The OP should try examining the circuit around that chip might reveal more detail and help further....

How do you know that this item is broken?

I will suggest that this is just a surface mount ceramic 5 resistor pack, probably with the same sizeresistors inside. May be 100E precision x 5. To know if any component in a circuit is wrong. You need a circuit diagram and a set of working voltages and also know where you can measure without doing more damage and on a RF circuit you need a dummy load for it to work. Better take this to someone with the knowledge to fix it. RF is like microwave, you do not see it and feel it and can burn you 2 inches deep or deeper if you do somethig stupid. Your health and the health of the people around you is worth much more than what a professional fix will cost.

Use the telephone to contact the board manufacturer directly.
