RCAS Repairing

We have got a 9" x 12" ball mill mounted on frame that is held down by RCAS anchor M30. the problem I am encountering is that the ball mill is now vibrating and the reason is the anchor bolts are loosening and coming out. How can I repair this and make sure that I secure this base frame. I did drill the concrete to increase the number of bolts but still these are also pulling out on tightening. Is there something Iam doing wrong. Curing time for the particular RCAS is 40 min but I have given them more time. All suggestions to help me solve this problem are welcome.
