Reminds me of a complaint I heard numerous times over the years, 'We never have time to do it right but we always have time to do it over again'.

What I have always told my people is, 'Worry about it ahead of time - after a problem shows it self it is too late'.

The system described was apparently not thought out before trying to commission it. A path toward disaster. Commissioning is something most plant people rarely do and routinely mess it up - just as the author did.

This is so true. So many times when a problem arises, panic mode takes over and random fixes are thrown at the problem creating more problems. It takes less time to sit and ponder over the causes with a team than to start throwing fixes around possibly creating more problems.

Good story. Good solution. If one can keep his head while others all around are losing theirs...

But...

Whatchawannabet that the next time the plant is taken down and different people (without direct knowledge of the startup incident) try to restart it, that the correct procedure will not have been documented and the outcome will likely not be as good!

When the cah-cah is about to hit the fan...pause...contemplate...review. then 3 deep breaths and make your move. you responded in time to save money and embarassment. would we all have? unfortunately my "pause" is about 24 hrs. !!

so do we need a psychologal leg to CR4?
